ABSTRACT

La inocuidad de la carne comercializada debe estar garantizada en la cadena de producción, para evitar enfermedades transmitidas por alimentos (ETA). Escherichia coli productor de toxina Shiga (STEC) y Salmonella spp. pueden encontrarse en el tracto gastrointestinal de los bovinos y contaminar la carne de consumo humano, pudiendo causar enfermedades en el hombre. Este trabajo tuvo como objetivo evaluar las condiciones higiénico-sanitarias de 52 carnicerías localizadas en Asunción y detectar la frecuencia de STEC y Salmonella spp. en muestras de carne molida. Las condiciones higiénico-sanitarias se evaluaron mediante la estimación del riesgo, utilizando una escala de clasificación por categorías. La detección de STEC y Salmonella spp. se realizó por PCR en tiempo real. En la evaluación inicial, se clasificaron a 33% de las carnicerías como de alto y moderado riesgo. Se detectó STEC no-O157 en un 50% (130/258) de las muestras y Salmonella spp. en un 11% (29/258). Se realizaron acciones de mejora. En la etapa post-intervención, no se detectaron carnicerías de alto riesgo. En el muestreo de seguimiento se detectó un 29% (66/237) de muestras positivas para STEC no-O157 y 7% (16 /237) para Salmonella spp. Este estudio permitió realizar recomendaciones específicas y detalladas a cada carnicería, lo que tuvo un efecto significativo en la mejora de sus condiciones. Esta situación resalta la importancia de continuar fortaleciendo la vigilancia multisectorial y multidisciplinaria. Es imperativo que los establecimientos que se dedican al rubro, implementen las Buenas Prácticas de Manufactura (BPM) como una medida para reducir los riesgos asociados.


The safety of marketed meat must be guaranteed in the production chain, to avoid foodborne illness. Shiga toxin-producing Escherichia coli (STEC) and Salmonella spp. can be found in the gastrointestinal tract of cattle and contaminate meat for human consumption, potentially causing diseases in humans. This work aimed to evaluate the hygienic-sanitary conditions of 52 butcher shops located in Asunción and detect the frequency of STEC and Salmonella spp. in ground beef samples. Hygienic-sanitary conditions were evaluated by estimating risk, using a categorical classification scale. The detection of STEC and Salmonella spp. was performed by real-time PCR. In the initial evaluation, 33% of the butcher shops were classified as high and moderate risk. STEC non-O157 was detected in 50% (130/258) of the samples and Salmonella spp. in 11% (29/258). Improvement actions were carried out. In the post-intervention stage, no high-risk butcher shops were detected. In the follow-up sampling, 29% (66/237) of positive samples were detected for STEC non-O157 and 7% (16/237) for Salmonella spp. This study allowed specific and detailed recommendations to be made to each butcher shop, which had a significant effect on improving their conditions. This situation highlights the importance of continuing to strengthen multisectoral and multidisciplinary surveillance. It is imperative that establishments dedicated to the sector implement Good Manufacturing Practices (GMP) as a measure to reduce associated risks.

(analítico) Los factores de riesgo y protección pueden ser utilizados para evaluar, prevenir e intervenir en el maltrato infantil. El objetivo de este estudio es analizar longitudinalmente el riesgo en la infancia y la adolescencia, para determinar qué factores pueden ser empleados como indicadores de evolución. Se empleó una muestra de 102 niños, niñas y adolescentes que participaban en el sistema de bienestar infantil español, en tres momentos temporales. Los resultados indicaron que existe una evolución del riesgo, con una tendencia general de disminución de riesgos e incremento de elementos protectores. Nuestros hallazgos identificaron la existencia de factores dinámicos y estáticos. Este estudio pone de manifiesto la relevancia de la atención e intervención continuada y a largo plazo para fortalecer los factores de protección y disminuir los de riesgo en niños, niñas y adolescentes.


(analytical) Risk and protective factors can be used for risk and needs assessments, prevention, and intervention in cases involving mistreatment of children. The aim of this study is to provide a longitudinal analysis of risks and vulnerabilities in childhood and adolescence, with the goal of determining which factors can be used as indicators for progress with cases. A sample of 102 children and adolescents supported by the child welfare system in Spain was used along with measurements based on three different points in time. The study's results indicated that there is a change in risk levels, with a general trend of decreasing risk and increasing protective elements. The study's findings identified the existence of a group of dynamic factors and another group of static factors. This study highlighted the relevance of continuous and long-term care and intervention in order to strengthen protection and decrease risks for children.


(analítico) Os factores de risco e protecção podem ser utilizados para avaliar, prevenir e intervir em casos de maustratos a crianças. O objectivo deste estudo é analisar o risco na infância e adolescência longitudinalmente, a fim de determinar que factores podem ser utilizados como indicadores de evolução. Uma amostra de 102 crianças e adolescentes participantes no sistema espanhol de bem-estar infantil foi utilizada em três pontos no tempo. Os resultados indicaram que existe uma evolução do risco, com uma tendência geral de diminuição do risco e aumento dos elementos protectores. Os nossos resultados identificaram a existência de factores dinâmicos e estáticos. Este estudo destaca a relevância dos cuidados e intervenções contínuos e a longo prazo para reforçar os factores de protecção e diminuir os factores de risco nas crianças.

ABSTRACT Introduction: Pre-apheresis peripheral blood CD34+ cell count (PBCD34+) is the most important predictor of good cell mobilization before hematopoietic stem cell transplantation, albeit flow cytometry is not always immediately available. Identification of surrogate markers can be useful. The CD34+ cells proliferate after mobilization, resulting in elevated lactate dehydrogenase (LDH) activity and correlating with the PBCD34+ count. Objective: To determine the LDH cut-off value at which adequate CD34+ cell mobilization is achieved and its diagnostic yield. Materials and methods: A total of 103 patients who received an autologous stem cell transplantation (ASCT) between January 2015 and January 2020 were included. Demographic and laboratory characteristics were obtained, including complete blood count, pre-apheresis PBCD34+ and LDH levels. Receiver operating characteristic (ROC) curves were performed to identify the optimal serum LDH activity cut-off points for ≥ 2 and ≥ 4 × 106 cells/kg post-mobilization CD34+ count and their diagnostic yield. Results: A post-mobilization serum LDH cut-off value of 462 U/L yielded a sensitivity (Se) = 86.8% (positive predictive value [PPV] = 72.7%), a pre- and post-mobilization serum LDH difference cut-off value of 387 U/L, an Se = 45.7% (PPV = 97%) and an LDH ratio of 2.46, with an Se = 47.1% (PPV = 97%) for an optimal mobilization count (CD34+ ≥ 4 × 106). Conclusion: The LDH measurement represents a fast and affordable way to predict PBCD34+ mobilization in cases where flow cytometry is not immediately available. According to the LDH diagnostic yield, it could be used as a surrogate marker in transplant centers, supporting the CD34+ count, which remains the gold standard.

ABSTRACT

Este artículo especial tiene el propósito de exponer el enunciado del Modelo de preparación para el cuidado desde la perspectiva de Enfermería, el cual se generó por la teorización y la reflexión problemática con base en los resultados de una investigación que tuvo como salida la tesis doctoral: Preparación a cuidadoras de casas de cuidado infantil, desde la perspectiva de Enfermería, del doctorado en Ciencias Biomédicas de la Universidad de Ciencias Médicas de la Habana, en su línea de Ciencias de la Enfermería en el año 2022. Se argumentaron los componentes del modelo y se exhibió su representación simbólica. Se concluyó que la preparación para el cuidado desde la perspectiva de Enfermería tiene una primera dimensión (acción dialógica profesional), y una segunda dimensión (reacción dialógica cognoscente de la persona que desarrolla el cuidado no profesional y del personal de Enfermería).


This special article has the purpose of exposing the statement of a Model to prepare for Nursing care, which was generated by the theorization and problematic reflection based on the results of an investigation that resulted in the doctoral thesis in Biomedical Sciences: Preparation for caregivers of child care homes, from the perspective of Nursing, developed at the University of Medical Sciences in Havana, in its line of research of Nursing Sciences during 2022. The components of the model and its symbolic representation were exhibited. We concluded that care preparation from the Nursing perspective has a first dimension (professional dialogical action), and a second one (cognizing dialogical reaction of both the person who performs the non-professional care and the Nursing staff).

ABSTRACT

Introducción: El hemangioma cavernoso es una de las neoplasias benignas más frecuentes en la infancia. Objetivo: Describir el caso de un hemangioma cavernoso en edad pediátrica. Caso clínico: Lactante femenina de 6 meses de edad, de procedencia urbana, cuyo nacimiento tuvo lugar en el Hospital Materno Fe del Valle Ramos, del municipio Manzanillo. Al nacer se observa una lesión en forma placa eritematosa violácea infiltrada de ± 10 cm, de borde definido, de superficie liza, con aumento de temperatura al tacto, que se extendía desde la parte medial anterior y posterior hasta la superior de la pierna del miembro inferior izquierdo. Se decidió interconsultar con el Servicio de Angiología, el cual diagnostica la lesión como hemangioma cavernoso. Conclusiones: Se corroboró que el hemangioma cavernoso es una entidad frecuente en la infancia, su diagnóstico y tratamiento oportunos son altamente beneficiosos para la mejoría y la cura, por lo que se impone la realización de un correcto y exhaustivo examen físico, de lo que se deriva también la prevención de complicaciones a corto, mediano y largo plazo(AU)


Introduction: Cavernous hemangioma is one of the most frequent benign neoplasms in childhood. Objective: To describe the case of a cavernous hemangioma at pediatric age. Clinical case: A 6-month-old female infant of urban origin was born at Hospital Materno Fe del Valle Ramos, Manzanillo Municipality, Granma Province. At birth, a violaceous erythematous plaque-like infiltrated lesion was observed, of approximately 10 cm, with a defined border, smooth surface, increased temperature at hand contact, extending from the anterior and posterior medial part to the upper leg of the left lower limb. It was decided to do an interconsultation with the angiology service, which diagnosed the lesion as a cavernous hemangioma. Conclusions: Cavernous hemangioma was corroborated to be a frequent entity in childhood, whose timely diagnosis and treatment are highly beneficial for improvement and cure, so it extremely necessary to carry out a correct and exhaustive physical examination, also deriving the prevention of complications in the short, medium and long terms(AU)

ABSTRACT

El cáncer de tiroides es una entidad infrecuente en población pediátrica, tiene un conjunto único de características clínicas, patológicas y moleculares en niños. La clínica típica es un nódulo tiroideo de meses de evolución asintomático, como en el caso descrito. En comparación con los adultos, los niños presentan con mayor frecuencia una enfermedad agresiva en etapa avanzada. La conducta de manejo y tratamiento es la cirugía de tiroides radical y el vaciamiento ganglionar amplio, luego yodoterapía y suplencia tiroidea. La supervivencia es excelente, a los 10 años es mayor a 98% sin embargo, se deben recordar las complicaciones relacionadas con el tratamiento de por vida las cuales no son infrecuente.


Thyroid cancer is a rare condition in the pediatric population, it has a unique set of clinical, pathological and molecular characteristics in children. The typical presentation is an asymptomatic thyroid nodule of months of evolution, as in the described case. Compared with adults, children more often present with late-stage aggressive disease. The management and treatment approach is radical thyroid surgery and extensive lymph node dissection, then iodotherapy and thyroid replacement. Survival is excellent, at 10 years it is greater than 98%, however, complications related to lifelong treatment, which are not uncommon, should be kept in mind.

ABSTRACT Introduction: Acute myeloid leukemia (AML) is most commonly presented in older adults; however, it appears 10 years earlier in Latin American countries. Clinical evolution in older adults from this populations has not been characterized. We analyzed outcomes and survival predictors. Methods: Patients ≥ 55 years old diagnosed with AML at a hematology referral center from 2005 to 2020 receiving intensive chemotherapy (IC), low-dose cytarabine (LDAC) and best supportive care (BSC) were included. Survival analysis included the Kaplan-Meier and Cox models and the cumulative incidence of relapse (CIR). Results: Seventy-five adults were included and the overall survival (OS) was 4.87, 1.67 and 1.16 months, using IC, LDAC and BSC, respectively. The IC led to a higher OS (p < 0.001) and was a protective factor for early death, at a cost of more days spent hospitalized and more non-fatal treatment complications; non-significant differences were found between the LDAC and BSC. Eight (10.7%) patients underwent hematopoietic cell transplantation, with a higher OS (p = 0.013). Twenty (26.7%) patients achieved complete remission; 12 (60%) relapsed with a 6-month CIR of 57.9% in those < 70 years old vs. 86.5% in those ≥ 70 years old, p = 0.034. Multivariate analysis showed the white blood cell count (WBC) and IC had a significant impact on the patient survival, whereas chronological age and the Charlson comorbidity index (CCI) did not. Conclusion: AML in low-middle income countries demands a different approach; the IC improves survival, even with a high incidence of relapse, and should be offered as first-line treatment. Eligibility criteria should include WBC and a multidimensional evaluation. The age per se and the CCI should not be exclusion criteria to consider IC.

Introducción: La búsqueda de la eficiencia en la atención a la salud de la población representa una prioridad para cada uno de los niveles de atención, por lo que es vital identificar los elementos que permitan una mejora continua en las instituciones. La gestión del conocimiento es utilizada para tal fin y se ha estudiado a la par de otras variables como: la cultura organizacional, el liderazgo y la disponibilidad y el uso de tecnologías de la información y comunicación. Objetivo: El objetivo del estudio fue analizar la relación de la gestión del conocimiento con estas variables en centros de atención primaria a las adicciones del noroeste de México. Métodos: Se aplicó un cuestionario (α = .977) a 80 profesionales de la salud que laboraban en esas instituciones. Se encontraron relaciones positivas, grandes y significativas entre la gestión del conocimiento y el liderazgo (r = .816, p <. 001) y entre la gestión del conocimiento y la cultura organizacional (r =. 802, p <. 001). Sin embargo, se encontró una correlación positiva y moderada con la variable tecnologías de la información y comunicación. Conclusiones: Se concluye que en los centros de atención primaria a las adicciones del noroeste de México la cultura organizacional y el liderazgo se consideran los factores más importantes para contribuir al éxito de la gestión del conocimiento, principalmente en la adquisición y la transferencia del conocimiento. Es importante considerar en estudios futuros los beneficios sociales y económicos de emplear la gestión del conocimiento en la toma de decisiones organizacionales.


Introduction: The search for efficiency in health care for the population signifies a priority for each level of care, so it is vital to identify the elements allowing continuous improvement in institutions. Knowledge management is used for this purpose and it has been studied along with other variables such as organizational culture, leadership and the availability and use of information and communication technologies. Objective: The objective of the study was to analyze the relationship of knowledge management with these variables in primary addiction care centers in Northwest Mexico. Methods: A questionnaire (α = .977) was applied to 80 health professionals who worked in these institutions. Positive, large and significant relationships were found among knowledge management and leadership (r = .816, p < .001) and among knowledge management and organizational culture (r = .802, p < .001). However, positive and moderate correlation was found with the information and communication technologies variable. Conclusions: It is concluded that in primary addiction care centers in Northwest Mexico, organizational culture and leadership are considered the most important factors to contribute to the success of knowledge management, mainly in the acquisition and transfer of knowledge. It is important to consider in future studies the social and economic benefits of using knowledge management in organizational decision making.

ABSTRACT

The COVID-19 pandemic has caused a large number of diseases worldwide. There are few vaccines to constrain this disease and the value of them is high. In this sense, the antigens of the vaccine platform Soberana, the receptor binding domain from SARS-CoV-2 Spike protein, both the monomeric (mRBD) and dimeric (dRBD) forms, have been developed. This study encompassed several analyses by different techniques like circular dichroism (CD), fluorescence spectroscopy (FS) and Gel Filtration- High Performance Liquid ChLC of mRBD and dRBD. Monomer and dimer exhibited similar far-UV CD spectral characteristics with 54% of β-sheet content. Similar conformational features according to near-UV CD and FS studies were observed in both RBD. Stress stability studies by far-UV CD, FS, biological activity and GF-HPLC at 37 °C showed that mRBD is very stable. On the other hand, dRBD fluorescent emission showed a shift towards higher wavelengths as the incubation time increases, suggesting exposition of tryptophan residues, unlike what happens with mRBD. Biological activity outcome confirms these results. GF-HPLC profiles showed that in mRBD, the product of molecular stress are dimers and does not increase over time. However, dRBD showed dimer fragmentation as the main degradation species. This study reveals the usefulness of CD techniques for the analysis of degradation of RBD molecules as well as showed the difference in stability of both RBD molecules. Besides, our work provides useful insights into the production of a key protein used in diagnosis and therapeutics to fight COVID-19 pandemia.

Resumen Introducción: El retinoblastoma (RB) es el tumor maligno primario intraocular más frecuente en la infancia y debe ser la principal patología a descartar en los pacientes pediátricos con leucocoria. La persistencia de la vasculatura fetal (PVF) está dentro del diagnóstico diferencial de la leucocoria, un trastorno del vítreo que surge de un defecto en la involución de la vasculatura hialoidea en la etapa embrionaria que afecta al desarrollo ocular normal y comúnmente produce microoftalmia asociada. El diagnóstico temprano y tratamiento oportuno son cruciales para un buen pronóstico y mejor esperanza de vida del infante. Caso clínico: Presentamos un caso de coexistencia de retinoblastoma y PVF: paciente de sexo masculino de 2 años y 11 meses sin reflejo rojo y vasculatura y membrana blanco-amarillenta detrás del cristalino en el ojo derecho. La ecografía en modo B con desorganización de la cavidad vítrea con ecos de alta reflectividad sugestivos de calcificación. A la exploración se observó globo ocular agrandado, rubeosis iridis, vasculatura posterior, presión intraocular 28 mmHg. Tomografía computarizada con masa intraocular heterogénea con regiones hiperdensas. Con diagnóstico de probable retinoblastoma, se realizó la enucleación. La histopatología informó el hallazgo de retinoblastoma moderadamente diferenciado coexistente con PVF. Conclusiones: El hallazgo de estos dos diagnósticos en un mismo ojo es muy poco frecuente debido a la diferente fisiopatología.


Abstract Background: As retinoblastoma (RB) is the most frequent primary intraocular malignant tumor in childhood, it should be the main pathology to rule out in pediatric patients with leukocoria. Persistence of fetal vasculature (PFV) is within the differential diagnosis of leukocoria, a vitreous disorder arising from a defect in the involution of the hyaloid vasculature in the embryonic stage, which affects normal ocular development and commonly produces associated microophthalmia. An early diagnosis and timely treatment are crucial for a better prognosis and life expectancy of the child. Case report: We present a case of retinoblastoma and coexisting with PFV: a 2-years-and 11-months-old male with no red reflex, and vasculature and yellowish-white membrane behind the lens of the right eye. B-mode ultrasound with disorganization of the vitreous cavity with high reflectivity echoes suggestive of calcification. On examination we found an enlarged eyeball, rubeosis iridis, posterior vasculature, intraocular pressure 28 mmHg. Computed tomography with heterogeneous intraocular mass with hyperdense regions. With a diagnosis of probable retinoblastoma, enucleation was performed. Histopathology reported moderately differentiated retinoblastoma coexisting with PFV. Conclusions: The finding of these two diagnoses in the same eye is very rare due to the different pathophysiology.

Con el objetivo de elevar el acervo cultural de estudiantes y profesionales nació en la Universidad de Ciencias Médicas de La Habana el proyecto científico-cultural Histartmed en el año 2005, hoy Centro de investigación sobre Historia, Arte y Medicina. A raíz de la participación de un grupo de profesionales y estudiantes de la Universidad de Ciencias Médicas de Villa Clara en el VIII Coloquio Integrador, surgió la cátedra honorífica Histartmed Villa Clara, fundada el 27 de mayo de 2014 por resolución rectoral del propio año. En el noveno aniversario de su creación, los autores se proponen exponer el impacto que esta ha tenido en el ámbito docente, extensionista e investigativo de la universidad.


SUMMARY With the objective of raising the cultural heritage of students and professionals, the scientific-cultural project Histartmed was born at the University of Medical Sciences of Havana in 2005, today the Research Center on History, Art and Medicine. As a result of the participation of a group of professionals and students from the Villa Clara University of Medical Sciences in the VIII Integrative Colloquium, the Histartmed Villa Clara honorary chair emerged, founded on May 27, 2014 by rectoral resolution of the same year. On the ninth anniversary of its creation, the authors propose to expose the impact that it has had in the teaching, extension and research fields of the university.

ABSTRACT

Fundamento: actualmente son elevadas las cifras de jóvenes con virus de inmunodeficiencia humana y síndrome de inmunodeficiencia adquirida, lo cual impone la necesidad de disponer de amplia información, percibir la vulnerabilidad propia y la gravedad del riesgo de infección. Objetivo: diseñar una propuesta de programa educativo con orientación popular para la modificación de percepción de riesgo del VIH/sida en estudiantes becarios de primer año de Medicina, 2021-2022. Métodos: se realizó una investigación de desarrollo, entre enero-diciembre de 2021. Se aplicaron métodos teóricos, empíricos y matemáticos. Se utilizó la variable percepción de riesgo, referida a la valoración del VIH/sida como amenaza para la salud de los seres humanos. Como instrumentos se manejó un cuestionario previamente validado. Resultados: el diagnóstico realizado teniendo en cuenta el nivel de información sobre el VIH/sida, vulnerabilidad y severidad percibidas, demostró insuficiencias en ambos sexos, con mayor frecuencia en el femenino; por lo que se diseñó un programa educativo basado en educación popular, estructurado en: título, introducción, justificación, objetivo general, metodología, evaluación y orientaciones de actividades, con el fin de modificar la percepción de riesgo del VIH/sida en la muestra. Conclusiones: fue valorado por un grupo de especialistas, quienes ofrecieron sugerencias para su optimización, aplicadas en su conformación definitiva, lo cual resultó de suma utilidad para el resultado de la investigación y su aplicación en contextos similares.


Background: currently there are high numbers of young people with human immunodeficiency virus and acquired immunodeficiency syndrome, which imposes the need to have extensive information, perceive one's own vulnerability and the severity of the risk for infection. Objective: to design a proposal for an educational program with popular orientation to modify the risk perception of HIV/AIDS in first-year medical scholarship students, 2021-2022. Methods: a development investigation was carried out from January to December 2021. Theoretical, empirical and mathematical methods were applied. The risk perception variable was used, referring to the assessment of HIV/AIDS as a threat to the health of human beings. A previously validated questionnaire was used as instruments. Results: the diagnosis carried out taking into account the level of information about HIV/AIDS, perceived vulnerability and severity, demonstrated insufficiencies in both sexes, more frequently in the female; Therefore, an educational program was designed based on popular education, structured in: title, introduction, justification, general objective, methodology, evaluation and guidelines for activities, in order to modify the perception of risk for HIV/AIDS in the sample. Conclusions: it was evaluated by a group of specialists, who offered suggestions for its optimization, applied in its final conformation, which was extremely useful for the results of the research and its application in similar contexts.

ABSTRACT

ABSTRACT It is clear that physical education is considered a compulsory subject and as such should be la-belled as an important subject, but this perspective is not always shared by the educational community. The aim of this study was to explore the perception of families with children enrolled in compulsory education about the subject of physical education. To this end, an ad hoc questionnaire was administered, consisting of 15 questions, two of which were socio-demographic in nature and the rest of which were Likert-type responses. The study is a non-experimental, descriptive, cross-sectional study. The study sample consisted of a total of 120 respondents (M=40.28 years; SD= 6.95). Participants were selected through non-probability purposive sampling. The results showed a lack of knowledge regarding the subject of physical education which implies that there are diverse opinions about the functioning and the proposed aim of this subject. However, physical education should be seen as an essential subject for the integral development of the student.


RESUMO É evidente que a educação física é considerada uma disciplina obrigatória e como tal deve ser rotulada como uma disciplina importante, mas esta perspectiva nem sempre é partilhada pela comunidade educativa. O objectivo deste estudo era explorar a percepção das famílias com crianças matriculadas no ensino obrigatório sobre o tema da educação física. Para o efeito, foi administrado um questionário ad hoc, constituído por 15 perguntas, duas das quais de natureza sócio-demográfica e as restantes respostas eram do tipo Likert-. O estudo é um estudo não-experimental, descritivo, de corte transversal. A amostra do estudo consistiu num total de 120 respondentes (M=40,28 anos; SD=6,95). Os participantes foram seleccionados através de amostragem sem fins lucrativos. Os resultados mostraram uma falta de conhecimento sobre o tema da educação física, o que implica que existem opiniões diversas sobre o funcionamento e o objectivo proposto para este tema. No entanto, a educação física deve ser vista como um assunto essencial para o desenvolvimento integral do estudante.

Resumen Introducción: La distribución espacial y temporal de la infección por SARS-CoV-2 sobrepasa las áreas endémicas de enfermedades transmitidas por vector (ETV), cuya vigilancia en México ha cambiado sustancialmente a partir del primer caso confirmado de COVID-19. Objetivos: Estimar y comparar las tasas de incidencia de las ETV antes y después de la introducción del SARS-CoV-2 en México. Métodos: Estudio retrospectivo de casos de ETV de 2014 a 2021. Las tasas de incidencia de cada ETV en el periodo previo (2014-2019) y posterior (2020-2021) a la introducción del SARS-CoV-2 en México fueron calculadas y comparadas. Resultados: Antes de la introducción del SARS-CoV-2, las tasas de incidencia de las ETV fueron altas y posterior a la introducción del coronavirus hubo un descenso en los índices epidemiológicos; sin embargo, solo se identificó diferencia estadística significativa en la tasa de incidencia de la malaria (p ≤ 0.05) y otras rickettsias (p ≤ 0.05). Conclusiones: Algunas medidas para reducir los casos de COVID-19, como el distanciamiento social, el confinamiento domiciliario, la reducción en el aforo en el transporte público y el trabajo en casa, probablemente contribuyeron a disminuir temporalmente el número de casos de las ETV; sin embargo, puede haber rebrote de las ETV en el futuro cercano.


Abstract Introduction: SARS-CoV-2 infection spatial and temporal distribution overlaps with endemic areas of vector-borne diseases (VBD), whose surveillance in Mexico has substantially changed since the first COVID-19 confirmed case. Objectives: To estimate and compare the incidence rates of VBDs before and after the introduction of SARS-CoV-2 in Mexico. Methods: Retrospective study of VBD cases from 2014 to 2021. The incidence rates of each VBD in the period before (2014-2019) and after (2020-2021) the introduction of SARS-CoV-2 in Mexico were calculated and compared. Results: Before the introduction of SARS-CoV-2, the incidence rates of VBDs were high and after the introduction of coronavirus there was a decrease in epidemiological indices; however, there was only statistically significant difference in the incidence rate of malaria (p ≤ 0.05) and other rickettsiae (p ≤ 0.05). Conclusions: Some measures to reduce COVID-19 cases, such as social distancing, home confinement, reductions in public transport and working at home (home office), probably temporarily decreased the number of VBD cases; however, there may be a resurgence of VBDs in the near future.

ABSTRACT

Abstract The aim of this investigation is to show the relationships between the use of Mobile Driving Applications and Devices (MDAD) with self-reported road accidents, anger, physical health, and subjective wellbeing, in three types of Colombian drivers. The sample is non-randomized (n = 1149) and 93.2% are male drivers. Results showed that (a) for professional drivers, there is an inverse correlation between the use of MDAD with lower driving frequency during weekdays and a higher educational level, (b) for occasional drivers, GPS use was directly associated with fatal road accidents and low driving frequency during weekdays, (C) for private drivers, correlations were found between MDAD broader usage, higher levels of both aggression and perceived driving difficulty as well as physical health problems mainly hypertension and diabetes. It is necessary to conduct in-depth research on MDAD uses, health, and driving styles, by controlling social desirability.


Resumen El objetivo de esta investigación es mostrar las relaciones entre el uso de Dispositivos y Aplicaciones Móviles para la Conducción (DAMC) con la accidentalidad vial auto reportada, la ira, la salud física y el bienestar subjetivo, en tres tipos de conductores colombianos. La muestra es no aleatoria (n = 1149) y el 93.2% son conductores varones. Los resultados muestran que (a) en conductores profesionales, hay una correlación inversa entre el uso de DAMC con una menor fre-cuencia de conducción entre semana y con un mayor nivel educativo, (b) en conductores ocasionales, mayor uso de GPS asociado con accidentes fatales y con menor frecuencia de conducción entre semana, (C) en conductores particulares, mayor uso de DAMC relacionado con niveles más altos de agresividad y de dificultad percibida para conducir, y con más problemas de salud -principalmente hipertensión y diabetes. Es necesario realizar investigaciones en profundidad acerca de los usos de DAMC, salud y estilos de conducción, mediante el control de la deseabilidad social.

Introducción: Los trastornos del sueño en niños, tienen un impacto en el desarrollo, afecta el comportamiento, el estado de ánimo, las funciones cognitivas, disminuyendo la atención selectiva y la memoria. El objetivo del presente estudio fue determinar las alteraciones del sueño en niños con trastorno del espectro autista (TEA), su afectación individual y familiar en el Centro Regional de Autismo Delicias Chihuahua. Métodos: El presente estudio transversal, incluye niños con TEA en quienes se determinó alguna alteración del sueño y se buscó su relación con la afectación individual y familiar, además se evaluaron, edad, sexo, grado escolar, nivel funcional de TEA, afectaciones familiares. Se aplicó el cuestionario de cribado del sueño en niños con TEA, el cuestionario adaptado para niños con TEA -BRUNI y el cuestionario de satisfacción sobre las actividades de los padres, cuidadores primarios y familiares de primera línea. Se usa estadística descriptiva y en un segundo análisis se usa Razón de Momios de Prevalencia (RMP). Resultados: Se analizan 57 pacientes, 43 (75.4%) hombres, 28 (49.1%) en grado 1 de nivel funcional TEA, 16 (28.1%) tenían mal desempeño individual, 56 (98.2%) sus familiares referían mal desempeño, 43 (75.4%) tenían alteraciones del sueño. Los niños preescolares tienen 2.5 veces riesgo de tener afec-tación individual. Los pacientes en nivel funcional TEA grado 3 tienen 2.15 veces riesgo de tener afectación individual. Los que despiertan cansados tienen 5.93 veces riesgo de tener afectación individual. Conclusión: La afectación familiar del desempeño es mucho mayor a la afectación individual de los niños con TEA, lo cual genera alteraciones en la dinámica familiar.


Introduction: Sleep disorders in children impact development, affecting behavior, mood, and cognitive functions and decreasing selective attention and memory. This study aimed to determine sleep disturbances in children with autism spectrum disorder (ASD) and their individual and family involvement at the Delicias Chihuahua Regional Autism Center. Methods: This cross-sectional study included children with ASD in whom some sleep disturbance was determined, and its relationship with individual and family affectation was sought; in addition, age, sex, school grade, functional level of ASD, and family affectations were evaluated. The sleep screening questionnaire for children with ASD, the adapted questionnaire for children with ASD -BRUNI, and the satisfaction questionnaire on the activities of parents, primary caregivers, and first-line family members were applied. Descriptive statistics were used, and the prevalence odds ratio (MPR) was used in a second analysis. Results: A total of 57 patients were analyzed; 43 (75.4%) were men, 28 (49.1%) were at the grade 1 ASD functional lev-el, 16 (28.1%) had poor individual performance, 56 (98.2%) their relatives reported poor performance, and 43 (75.4%) had sleep disturbances. Preschool children have a 2.5 times higher risk of having individual involvement. Patients in functional level ASD grade 3 have a 2.15 times higher risk of having individual involvement. Those who wake up tired have a 5.93 times higher risk of having individual involvement. Conclusion: The family affectation of performance is much greater than the personal affectation of children with ASD, which generates changes in family dynamics.

ABSTRACT Background: The date of acute lymphoblastic leukemia (ALL) diagnosis has been studied regarding potential etiologic roles with contrasting results and the issue remains controversial. The principal aim of this study was to analyze monthly variation of ALL diagnosis in a large homogenous Hispanic Latin American cohort over 15 years; its association with survival rates was also assessed. Methods: Clinical files and electronic records of 501 consecutive patients of all ages with ALL in northeastern Mexico over the years of 2004-2018 were scrutinized. Patients were divided into children <18 and adults >18 years. The Chi-square heterogeneity analysis was used to test for non-uniform variation. The Poisson regression analysis was used to fit sinusoidal (harmonic) models to the data, using the month of diagnosis as a covariate in a separate model. Results: During the study period 363 children (72.5%) and 138 adults (27.5%) (p < 0.001) were diagnosed with ALL. Heterogeneity across the months of diagnosis was confirmed (p = 0.019) and the Poisson regression analysis confirmed a significant monthly variation (p < 0.001) (95% CI, 3.024-3.745), a higher annual peak being observed in the month of March (p = 0.002), followed by a second peak in October (p = 0.026). The five-year OS for children was 68.2% (95% CI, 67.64-68.74) and for adults, 43.7% (95% CI, 42.67-44.71) (p < 0.001). No significant association between the month of diagnosis and OS was found (p = 0.789). Conclusion: The monthly variation of ALL diagnosis was documented; these results confirm the heterogeneous behavior of the disease and appear to be consistent with an interplay of environmental and biologic factors. Further studies are needed to examine putative candidate agents.

ABSTRACT

RESUMEN Los organoides son estructuras miniaturizadas, generadas principalmente a partir de células madre pluripotentes inducidas, que se cultivan en el laboratorio conservando sus características innatas o adquiridas. Tienen el potencial de reproducir procesos de desarrollo biológico, modelar procesos patológicos que permitirán el descubrimiento de nuevos fármacos y propicien la medicina regenerativa. Sin embargo, estas experiencias requieren perfeccionamiento constante porque pueden haberse realizado variaciones en la constitución de estos órganos. Por ello, el presente artículo tiene como objetivo revisar la información actualizada sobre organoides y sus procesos experimentales básicos y recientes, empezando por la gastrulación, para tratar de imitar, en lo posible, la formación de las tres capas: ectodermo, mesodermo y endodermo, incluyendo los factores que intervienen en la inducción, diferenciación y maduración en la generación de estos organoides. Asimismo, el diseño y preparación de medios de cultivo altamente especializados que permitan obtener el órgano seleccionado con la mayor precisión y seguridad. Se realizó una búsqueda de artículos originales y de revisión publicados en PubMed, Nature y Science. Los artículos se seleccionaron por sus resúmenes y por su texto completo. Las conclusiones de este articulo destacan las ventajas futuras en el uso y aplicaciones de los organoides.


ABSTRACT Organoids are tiny structures, mainly generated from induced pluripotent stem cells, which are cultured in the laboratory while retaining their innate or acquired characteristics. They have the potential to reproduce biological development processes, model pathological processes that will enable the discovery of new drugs and promote regenerative medicine. However, these processes require constant improvement because variations may have occurred in the constitution of the organs. Therefore, this article aims to review updated information on organoids and their basic and recent experimental processes, starting with gastrulation, in an attempt to mimic, as much as possible, the formation of the three layers: ectoderm, mesoderm and endoderm; as well as the information regarding the factors involved in the induction, differentiation and maturation during the generation of organoids. Likewise, the design and preparation of highly specialized culture media that allow obtaining the selected organ with the highest precision and safety. We searched for original and review articles published in PubMed, Nature and Science. Articles were selected for their abstracts and full text. The conclusions of this article highlight the future advantages in the use and applications of organoids.
